Use our pre-printed referral cards with a QR code, send a fax, submit an e-referral, or call us directly. Each referral card is practice-specific, so your referrals are tracked accurately from day one. Need cards for your office? We’ll deliver them.
Within 24 hours of receiving your referral, our team contacts your patient directly to complete intake and schedule their first visit. Your office staff doesn’t need to coordinate anything beyond the initial referral.
Your patient’s first evaluation happens at home — in the environment where they’re actually recovering. A personalized treatment plan is built immediately, and structured progress updates start flowing back to your office at the milestones you care about.
We Treat Your Referral as a Partnership, Not a Transaction
Here’s what you can expect:
*2-week update — Early progress indicators.
*6-week update — Functional milestone progress, any clinical concerns, and projected timeline to discharge.
*12-week update (or at discharge) — Final outcomes summary, recommendations for continued activity or follow-up, and discharge status.
